Evidence-Based Complementary and Alternative Medicine 2016, 1-11 doi:10.1155/2016/3587569 In this study researchers aimed to explore acupuncture as an opportunity to promote natural fertility by undergoing a course of both acupuncture and lifestyle modifications, when compared to an active control group which underwent lifestyle modification alone. Those receiving the acupuncture conceived within an average of 5.5 weeks compared to 10.67 weeks for the lifestyle only group. The women who received the acupuncture protocol experienced an increase in their fertility awareness, wellbeing, improved menstrual cycles, and conceived in half the time. MECHANISMS Cochrane S, Smith CA, Possamai-Inesedy A, Bensoussan A. Acupuncture and women's health: an overview of the role of acupuncture and its clinical management in women's reproductive health. International journal of women's health. 2014;6:313-325. This study shows that acupuncture may impact fertility, ovulation, and menstrual regularity by triggering neurotransmitters which in turn increase GnRH, acupuncture may stimulate blood flow by inhibiting central sympathetic nerve activity, and acupuncture may stimulate production of endogenous opioids, which may inhibit the central nervous system’s outflow and the biological stress response. IMPROVEMENT IN BLOOD FLOW Acupuncture improves uterine lining. Napadow et al, 2008 wrote that: “acupuncture regulates uterine and ovarian blood flow (OBF), and that the effect is most likely mediated as a reflex response via the ovarian sympathetic nerves, and that the response is controlled via the supraspinal pathways.” Stener-Victorin et al, 1996; Stener-Victorin et al, 2006; Yu et al, 2007. ACUPUNCTURE + MALE FERTILITY Recent studies have shown that acupuncture can effectively increase sperm production, improve sperm movement (motility, increase the percentage of healthy sperm, improve the levels of hormones responsible for fertility and increase the rate of pregnancy when used in conjunction with ART. Pei J et al. Quantitative evaluation of spermatozoa ultrastructure after acupuncture treatment for idiopathic male infertility. 2005 Jul;84(1):141-7 Fertility and Sterility. Forty men with at least one of three problems with their sperm were treated with acupuncture twice a week for 5 weeks. A statistically significant increase after acupuncture in the percentage and number of sperm with no structural defects was demonstrated compared to the control group of patients who received no treatment. They concluded that male infertility patients could benefit from having acupuncture. Dieterle et al. A prospective randomized placebo-controlled study of the effect of acupuncture in infertile patients with severe oligoasthenozoospermia. Fertility and Sterility, 2009 Oct. 28 infertile patients with severe oligoasthenozoospermia (high percentage of poorly swimming sperm ie poor mobility) received acupuncture according to the principles of traditional Chinese medicine (TCM) and 29 infertile patients received placebo acupuncture. The men in the study who received acupuncture had significant improvements in the percentage of sufficiently motile sperm. Gurfinkel E et al. Effects of acupuncture and moxa treatment in patients with semen abnormalities. Asian J Androl. 2003 Dec;5(4):345-8. A prospective, controlled and blind study put two randomized groups of infertile men into an acupuncture group and a control group. After 10 treatments, the patients in the acupuncture group demonstrated a significant increase in the percentage of normal forms compared to the control group. ACUPUNCTURE DOSAGE Stener-Victorin, 1996. Acupuncture improved OBF after 8 treatments. Jedel et al, 2011. Low frequency electro-acupuncture (EA) decreased hyperandrogenism and regulated menstrual cycles after 16 weeks of treatment. Johansson et al, 2013. Acupuncture increased ovulation frequency in PCOS patients with 10-14 weeks of treatment 2x/week. Hullender Rubin et al, 2014; Magarelli et al, 2009. Acupuncture improves live births when administered at a larger dose (9-12 visits prior to ET). Manheimer et al 2013; Cheong et al, 2013. Acupuncture performed on on the day of ET does not appear to benefit IVF outcomes when compared to any controls at this time. However, compared to a “no acupuncture” control, we do see benefits. (Any acupuncture is better than no acupuncture!) ACUPUNCTURE + IVF Paulus W et al, Fertil Steril (2002). Influence of acupuncture on the pregnancy rate in patients who undergo assisted reproduction therapy, Fertility and Sterility;Vol 77 721-724. 160 patients with good quality embryos underwent IVF. They were divided into the following two groups through random selection: embryo transfer with acupuncture, and embryo transfer without acupuncture. Acupuncture was performed for 25 minutes before and after embryo transfer. In the control group, embryos were transferred without any supportive therapy. Clinical pregnancies were documented in 42.5% in the acupuncture group, whereas pregnancy rate was only 26.3% in the control group. Hullender Rubin Lee, et al. (2015). Impact of whole systems traditional Chinese medicine on in vitro fertilization outcomes. Reproductive Biomedicine Online; pii:S1 472-6483(15)00092. In this study, 1231 IVF patient records were assessed and outcomes were compared. They compared IVF with no treatment, IVF with just acupuncture, and IVF with Whole Systems Traditional Chinese Medicine (WS-TCM). (WS-TCM is what practiced at FMIM, and is an intervention of 12 acupuncture sessions before IVF, herbal medicine, diet, and lifestyle recommendations). The primary outcome was live birth. WS-TCM was associated with greater odds of live birth at 61%, compared with IVF alone (48%), or embryo transfer with acupuncture only (51%). WS-TCM was associated with increased live births compared with all groups, in donor and non-donor cycles. Balk J et al, Compl Therapies in Clinical Practice (2010). The relationship between perceived stress, acupuncture, and pregnancy rates among IVF patients: A pilot study, Complementary Therapies in Clinical Practice 16,154–157. These researchers at a university IVF clinic in Pittsburgh were interested to investigate the relationship between acupuncture, stress and pregnancy rates. The patients who received acupuncture on the day of embryo of transfer had a pregnancy rate of 55.6% compared with the control group pregnancy rate of 35.5% Dalton-Brewer N et al (2010). Uses of Acupuncture Before and After Embryo Transfer, Human Fertility, Vol 12 No 4 212 – 255. This report describes outcomes for all patients who attended the London Bridge Fertility, Gynecology and Genetics Centre in London over a 2-year period and who had acupuncture. In the acupuncture group positive pregnancy rates/ET were 44.6% comparing favorably with the non-acupuncture historic control group. When they analyzed outcomes in different age groups they discovered that acupuncture intervention was particularly effective in woman in the 35 - 39 and the over 40 group. STRESS + IVF + ACUPUNCTURE Acupuncture significantly reduces stress and anxiety of IVF. ACUPUNCTURE SAFETY Acupuncture has been extensively studied, and research consistently supports its safety when performed by trained practitioners. Numerous clinical trials and systematic reviews have shown that acupuncture carries a low risk of adverse effects, with the most common being minor issues like bruising or soreness at the needle site. Serious complications are rare, especially when the procedure is conducted with proper technique. As with any treatment, it's important to ensure that acupuncture is performed by a licensed, experienced professional to minimize any risks. It’s important to note, however, that in some states, healthcare providers like physicians, physical therapists, and chiropractors may practice acupuncture without a specific acupuncture license. Often referred to as "dry needling," this technique is essentially a form of acupuncture but with far fewer training hours and less extensive study. Most injuries, including rare cases of pneumothorax (lung collapse), associated with acupuncture are not typically linked to licensed acupuncturists, but rather to those with less specialized training in needling techniques. Regular acupuncture and Maya Abdominal Therapy sessions prior to IVF or IUI help reduce stress, increase uterine and ovarian blood flow, and prepare the body for conception. These therapies, along with herbal medicine when appropriate, can support hormonal balance, encourage healthy follicle development, and create optimal conditions for implantation. In preparation for the embryo transfer phase, acupuncture and Maya Abdominal Therapy may further enhance uterine receptivity by promoting circulation, calming the nervous system, and lowering cortisol levels—the body’s primary stress hormone. This relaxed, well-nourished state can improve implantation and early embryo development. Patients who incorporate these therapies throughout their fertility journey often experience more balanced hormone levels, improved responses to stimulation medications, higher fertilization and pregnancy rates, and reduced miscarriage risk. Treatment sessions will often be coordinated with each stage of the reproductive cycle to support the body’s natural rhythm. Preparation with acupuncture should be scheduled carefully to have the best chance of success. Some studies show more than double the chances of conception for couples utilizing acupuncture alongside IVF. Recent clinical studies suggest that acupuncture and Chinese herbal treatments may: balance reproductive hormones improve sperm (count, morphology, &amp; motility) improve blood flow to the ovaries &amp; uterus increase implantation rates &amp; pregnancy rates enhance egg quality increase ovarian response increase uterine lining thickness and enhance uterine receptivity reduce stress, anxiety, and depression associated with infertility prevent miscarriage and early pregnancy loss decrease western drug &amp; medication side effects Acupuncture and TCM herbal medicine help to mitigate side-effects from drugs and medication, as well as issues of circulation, inflammation, and other blockages (such elevated stress hormones) that may have been preventing you from conceiving and carrying to term in previous attempts at conception and pregnancy, whether naturally or using ART. Before an IVF Cycle Generally speaking, it is recommended to meet with both partners 1-2 times a week for at least 12 weeks prior to an IVF/IUI cycle, partly due to the fact that around 120 days is how long it takes for both an egg and sperm to go through an entire development cycle. At Fertile Moon Integrative Medicine, we use a whole-systems TCM approach which is highly individualized, so the exact combination of interventions and the dosage of said interventions will be on a case-by-case basis. For couples with complex or especially challenging health situations, six months would be even better. Remember that important changes take time. During this time, we will focus on general health and wellbeing, increasing both partners’ reserves, and fine-tuning blood circulation. Starting treatment prior to fertility medications also opens up the possibility of utilizing herbal medicine alongside acupuncture, which can be crucial for some people. Priming Phase During the priming phase, it is generally recommended to continue weekly acupuncture, as well as regular Maya Abdominal Therapy sessions. For most cycles, women take medications to temporarily flare and/or suppress the ovaries in preparation for stimulation. Stimulation Phase Stimulating medications are used to enhance the growth of eggs in the ovaries, and during this time it is recommended to continue with weekly or twice weekly acupuncture, as well as regular Maya Abdominal Therapy sessions. This phase can vary in length from cycle to cycle and will be monitored closely. 1-2 days Prior to Retrieval Once ovarian follicles are at the correct stage and size, you will be given a trigger to complete egg maturation in preparation for retrieval. Acupuncture at this time is very important to help with your eggs’ final maturation and should be done within the day or two prior to retrieval. After Retrieval After your eggs have been retrieved, you may feel some cramping, bloating, and discomfort from the procedure. The retrieval is an invasive procedure and acupuncture will be very useful to stimulate the healing process and prepare your uterus for implantation. At this time, acupuncture points (with or without moxibustion) will be used to relax the uterus, repair micro-trauma by increasing blood circulation and decreasing inflammation, and prepare your body for embryo transfer. Before Transfer It is strongly recommended to schedule a combined acupuncture and Maya Abdominal Therapy session 24–48 hours prior to your IUI or embryo transfer. These treatments work synergistically to enhance uterine blood flow, improve lymphatic and nerve function, and regulate the body’s stress response—creating an optimal physiological environment for implantation and early embryo development. After Transfer After your IUI/IVF procedure, it is usually recommended to follow up with 2-3 acupuncture treatments within the next 5-7 days post-transfer, in order to further assist implantation and embryo development. Maya Abdominal Therapy is usually not recommended at this time, unless modified by a professional. The evidence that we have suggests that acupuncture immediately prior to and after an embryo transfer may increase the odds of a successful pregnancy. Beyond Transfer After your IUI or embryo transfer procedure and at least through the end of the first trimester if there is a pregnancy, it is recommended to continue with weekly acupuncture to support a healthy pregnancy. Acupuncture treatments twice weekly for at least 12 weeks is ideal if there is a history of miscarriage or in an otherwise high-risk pregnancy. If the IUI or embryo transfer is unsuccessful, acupuncture and Maya Abdominal Therapy can offer valuable support for both the body and the heart. These therapies help regulate hormones, re-establish pelvic and uterine balance, and encourage healthy blood and lymph flow to clear stagnation and prepare the body for future cycles. Emotionally, they promote deep relaxation, stress relief, and nervous system regulation, helping you process disappointment and reconnect with a sense of grounding and hope.</image:caption>
